Use this Letter Request Form below to request a letter of good standing, a letter of permission for registered parishioners or a boundary letter if you live within the boundaries of our Parish.

If you are a Godparent for Baptism or a Sponsor for Confirmation, our Letter Request Form below is NOT what you need. You must take the “Archdiocese Certificate of Eligibility, also known as the Sponsor Form” to your Parish where you attend mass. Please ask your Pastor to sign and seal this form.
